Insurance & Billing Policy

We strive to participate with most insurance plans. We ask that you verify your coverage with your insurance company prior to your visit. At each visit, you will be asked to present your insurance cards to insure proper billing. Please be aware, however, that there are some costs not covered by all insurances. These costs may include:

  • Co-Pays – Most insurances require you to pay a small fee at your visit.
  • Deductibles - Many insurances require you to pay up to a certain amount each year before benefits start.
  • Refraction – This measurement for eyeglass lenses is not considered a medical service and may not be covered by insurance - the fee is $25.00. Some vision plans do cover this.
  • Other non covered services – These are services which insurances do not typically cover. Some of these services include contact lens services, Laser Vision Correction (LASIK and PRK), Presbyopia-correcting lens implants, and Toric Lens implants or other procedures to reduce astigmatism at the time of cataract surgery.

Vision Coverage

Some insurance include a "Vision eye exam" or "Vision coverage" benefit. Please check to see whether your insurance includes this benefit. If so, we will need to know how often you are eligible for this benefit (e.g. every 12-24 months or within a calendar year) and the last time it was used. If your carrier requires you to have a Vision Care benefit form you must present this form to the receptionist upon check-in or you will be financially responsible for payment at the time of your visit. This benefit can be used to cover basic exams for glasses and contact lenses. However, examinations which are meant to check on a medical eye condition, such as cataracts, glaucoma, diabetic eye exams, etc., will use medical insurance. A referral from your primary care doctor is required by a few plans.

Referrals

Your insurance may require a referral from your primary care doctor to authorize insurance coverage. If so, we urge you to call your primary doctor's office for this referral in advance. Without the required referral, you would be responsible for the exam fee and any other associated service/procedure fees.
